Lutheran Family Services (LFS) is expanding our impact—and we’re hiring compassionate, dedicated professionals to join our new Assertive Community Treatment (ACT) team in Fremont, Nebraska. This is an opportunity to be part of a collaborative, mission-driven program that provides life-changing mental health support to individuals in our community.
What Is Assertive Community Treatment (ACT)
Assertive Community Treatment (ACT) is a proven, multidisciplinary, team-based approach to mental health care. It provides community-based services to individuals living with serious mental illness many of whom also face challenges such as substance use, frequent hospitalization, homelessness, involvement with the criminal legal system, or psychiatric crises.
Catherine Fletcher, Director of Program Development with Lutheran Family Services, said, “The array of services and the flexibility in how services are delivered significantly impacts clients and their recovery experience.”
Why Work on an ACT Team at LFS?
At LFS, we believe meaningful change happens through relationships, teamwork, and dignity-centered care. Our ACT program is grounded in collaboration and shared responsibility—no one works alone, and clients are supported by the entire team.
Fletcher mentioned the benefits of joining the ACT team are two-fold. She said, “There’s the pragmatic side – competitive compensation and hiring bonuses, and then there are the perks of meaningful work, impacting lives, and being part of a collaborative team.”
As an ACT team member, you will:
- Work alongside professionals from diverse disciplines who share a commitment to recovery-oriented care
- Provide flexible, proactive, and consistent support tailored to each individual’s needs
- Be part of a program that values open communication, shared leadership, and continuous learning
- Help clients reduce crises, increase independence, and build skills for long-term stability
We foster a workplace culture that honors diversity, values staff voices, and creates a safe, supportive environment for both employees and clients.
What Makes ACT Unique: Meeting Need In The Community
Unlike traditional office-based care, ACT teams meet people where they are—at home, in the community, or wherever support is needed. By offering coordinated, flexible services, ACT helps individuals build stability, recover, and thrive in their own communities
ACT follows evidence-based principles that support both clients and staff:
- Small Caseloads: Low client-to-staff ratios allow for meaningful, individualized care.
- Shared Caseloads: The entire team is responsible for client success, offering continuity and balance.
- Multidisciplinary Collaboration: Mental health professionals, nurses, case managers, and specialists work together seamlessly.
- Daily Team Coordination: Teams meet daily to stay connected and respond quickly to changing needs.
- Person-Centered Planning: Clients guide their own goals, and teams support them in achieving what matters most.
Our core values of empathy, patience, resilience, flexibility, communication, and reliability guide how we show up for one another and for the people we serve.
Supporting Recovery at Every Stage
ACT provides structured, comprehensive care across the full treatment journey—from engagement and assessment to active treatment, rehabilitation, crisis response, and transition planning. This approach reduces hospitalization and homelessness while promoting independence, skill-building, and long-term recovery.
For staff, this model offers clarity, strong team support, and the opportunity to see real impact over time.
