The client portal will help you navigate your care at Lutheran Family Services.

  • Client portal will provide clients access to update forms, manage appointments, and access appointments virtually.

• Request Medical Records

  • To request your records email Medical Records at medicalrecords@onelfs.org
  • Processing Your Request
    • In most cases, record copies are available in less than a week, however some requests may take longer to process.
    • If you have questions or need to verify the status of your submitted request, please contact us at medicalrecords@onelfs.org or (402) 441-7940.
