Non-Intensive Case Management
Extended Case Management (ECM) at Lutheran Family Services provides non-intensive case management services to eligible clients. These services include support with public benefits such as:
- SNAP applications
- SNAP recertification
- Medicaid applications
ECM also connects clients to community resources, particularly for those experiencing financial need.
Additional Case Management Services
Additional services offered through ECM include assistance with:
- Food resources
- Housing
- Utilities
- Social Security applications and follow-up
- Address Changes
- IOM payment setup
- Referrals to Employment and Immigration Legal Services as needed.
