Extended Case Management

Non-Intensive Case Management

Extended Case Management (ECM) at Lutheran Family Services provides non-intensive case management services to eligible clients. These services include support with public benefits such as:

  • SNAP applications
  • SNAP recertification
  • Medicaid applications

ECM also connects clients to community resources, particularly for those experiencing financial need.

Additional Case Management Services

Additional services offered through ECM include assistance with:

  • Food resources
  • Housing
  • Utilities
  •  Social Security applications and follow-up
  • Address Changes
  • IOM payment setup
  • Referrals to Employment and Immigration Legal Services as needed.
